Result reporting process
In Mikrochem laboratories the examination results may be delivered in form of:
- printed paper report,
- electronic report while printed paper report is sent subsequently,
- phone announcement while printed paper report is sent subsequently.
Printed paper report
Final results are delivered in form of written reports typed in LIMS. Prior to releasing, the results are always confirmed by responsible university graduate lab employee. The proper confirmation and releasing of report is noticed in form of competent lab employee‘s signature next to the relevant result.
Results may be delivered either by drivers of delivery services, courier services or postal services.
Partial and preliminary results are announced only by phone (see 5.1.3).
Electronic report
Final results in electronic report have similar form as printed paper reports typed in LIMS including the confirmation by competent university graduate lab employee (see 5.1.1). Results reported electronically are always consequently delivered in printed paper form too.
Phone announcement of results
To speed up the reporting process, partial and preliminary results may be on request announced by phone.
The reports may be announced by phone only to a competent person, i.e. applicant - medical doctor or applicant‘s medical staff. Results are also communicated to the patient, in case of minors to their delegates. Prior to result reporting, the lab employee must verify recipient's identity (personal No.) and the phone announcement including the date and recipient's name are entered into LIMS. All technical lab staff is authorized to announce results by phone. In case the results are equivocal or the applicant demands a consultation, only university graduate lab employees are authorized to announce results. Results are consequently delivered to the applicant-doctor in form of printed paper report or electronic report.
Result reporting to patients
Concerning private payers - if the examination was performed on direct request of the patient without any application form from medical doctor, the final results are always reported directly to the patient. If the examination was performed on account of application form from medical doctor, the final results are reported directly to the patient only if such demand is explicitly mentioned on the form.
Otherwise, the results are reported only to the applicant-doctor.
The copy of the result may be given to the patient after identity verification.
Result and finding revisions
Correction or revision of results may be carried out only by competent university graduate lab employee who is obliged to refer incorrect result to the applicant-doctor immediately after its detection. Competent lab employee also informs the applicant-doctor about the means of result correction.
If the result has not been reported yet in electronic or printed form, nevertheless the incorrect result has been already announced by phone, the competent university graduate lab employee is obliged to immediately report revision of result by phone and enter a record into LIMS.
Every result or finding revision must be indicated with date, time, name of the employee who carried out the revision and grounds for revision. New result must be clearly indicated as "revised".
Previous incorrect results in printed paper report must stay legible. Revised report must be subsequently enclosed to the previous report, so that the result revision can be detected at first sight.
